Information Letter on Organizational Survey
Study Title: Evaluating system transition to virtual child and youth mental health services in response to COVID-19
Principal Investigator: Dr. Evangeline Danseco at email@example.com
What is this study? Children’s Mental Health Ontario (CMHO) and the Ontario Centre of Excellence for Child and Youth Mental Health (the Centre) are working together to evaluate the shift in virtual care in the child and youth mental health sector, in response to the COVID-19 pandemic. We are reaching out to agency leaders, service providers, youth and families across Ontario to learn about their experiences in this process.
Why is this study being done? We are conducting this study to identify facilitators and challenges to implementing victual care. We would like to use this information to carefully plan the steps to add virtual care options to child and youth mental health services post-pandemic.
Do I have to take part? Taking part in this study is voluntary. You may choose to stop participating at any time.
What will I need to do to take part? We will ask you to complete a brief survey on SurveyMonkey to share your experience in transitioning your organization towards providing virtual care during the physical restrictions resulting from the COVID-19 pandemic.
How will my information stay private? Your name and your organization’s name will be kept in a separate list from responses. These will be kept in a password-protected file and not be included in data for analysis. Data will instead use organization and participants IDs. Your answers will be kept private and secure and will not be linked to your personal information. We will keep all the responses to the surveys on secure servers in Canada (as per the SurveyMonkey agreement). The surveys will be kept for 7 years after the study has ended, then they will be deleted.
What are the benefits of the study? You may or may not benefit directly from the study. However, the information you will share with us may be used to make changes and provide better virtual mental health services to young people and their families across Ontario.
What are the risks of the study? We do not envision any significant risks related to participation in this study.
How do I agree to take part in the study? You are providing your consent by completing the survey.
