Request a Foundational Family Services Appointment

Welcome to Maltby Centre – you’re in the right place!

Foundational Family Services are available free of charge to all families registered in the Ontario Autism Program. Services may include family and peer mentoring, caregiver workshops and follow-up coaching sessions, brief targeted consultations, transition supports and family resources and clinic days.  You can choose to access these services at any point based on your needs, at no cost. There are no limits to the number of times you can register for these services.

Once we receive your referral, you will get an email with your appointment time within two business days.
